ID, Insurance Card, Doctors Referral (if your insurance requires it).
Please have your online paperwork completed or arrive early. The PT needs to review it before beginning your appointment.

Comfortable and Loose-Fitting Clothes: Think athletic wear, yoga pants, shorts (if your issue is in the lower body), t-shirts, and tank tops. These types of clothes won't restrict your movement during exercises.
Easy Access to the Treatment Area
Supportive Shoes: Wear comfortable, supportive athletic shoes or sneakers. Avoid open-toed shoes, sandals, flip-flops, or high heels, as they don't provide the necessary stability for exercises. If your therapy involves a foot or ankle issue, wear the sneakers you use most often. Bring socks, as you may be asked to remove your shoes for certain exercises.
There are private changing areas if needed.

The initial visit typically lasts 1 hour. Please complete registration prior to arrival.
Follow up visits are 45 min to 1 hour.
